The attack, which occurred approximately 40 nautical miles south of the port city of Galle, is leaving 101 sailors missing and 78 others injured, they report. It was the first time in decades that a submarine has successfully destroyed a surface naval ship in an act of war, representing a potentially deadly new stage in five-day-long fighting between Iran and the U.S.-Israeli coalition.

The Attack and Sinking

The IRIS Dena, a modern Moudge-class frigate and a pride of the Iranian Navy’s Southern Fleet, was said to be returning from the MILAN 2026 multinational naval exercises in Visakhapatnam, India, when it issued a frantic distress signal at dawn. The ship suffered a large underwater explosion that damaged its hull, according to sources from Sri Lankan naval and defense ministries. Just outside Sri Lankan territorial waters, the frigate rapidly sank. Though the submarine's identity hasn’t been announced, opposition lawmakers in the Sri Lankan Parliament have questioned the proximity of U.S. and Israeli naval assets currently working under Operation Epic Fury.

Rescue Efforts and Casualties

The Sri Lanka Navy and Air Force then immediately began a joint search and rescue mission, deploying two naval vessels and a Beechcraft aircraft to the area.

  • The Survivors: So far 32 sailors have been dragged from the water, many said they were in “critical condition” with serious burns and blast injuries.
  • Medical Response: The rescued personnel were taken to the Karapitiya Teaching Hospital in Galle, which has been placed on high alert.
  • The Missing: Sri Lankan Foreign Minister Vijitha Herath told Parliament that the potential for locating more survivors is “diminishing rapidly” because of choppy sea conditions which resulted in 101 crew members being unaccounted for out of 180 aboard.

A Global Flashpoint in the Indian Ocean

When the IRIS Dena goes down, it represents a catastrophic event in the Iranian navy, as the vessel was commissioned only in 2021 and was already considered an integral part of an Iranian maritime defense strategy. A US Central Command (CENTCOM) made no specific comment regarding the situation in the Sri Lanka operation, but recently reported the destruction of 17 Iranian naval vessels since the start of the attacks on 28 February.

The strike may have been initiated by a nuclear-powered attack submarine that operated out of a U.S. base in Diego Garcia. Geopolitical Fallout. From their standpoint, the strike puts Sri Lanka on a slippery playing ground in today's world political environment.

However, while the government still states that its rescue mission is only one way it meets all of its international maritime obligations--a "hot" warzone right on its southern shore has sparked a public panic in the local shipping industry. The Indian Ocean, which once lay in the shadow of the war but had now become a battlefield, is now home to one of Iran's most advanced warships. This gives cause for doubt that the dispute might continue to spill over into other world major trade routes.
